State Form W-4

Many states have addition state tax withholding forms that they use in conjunction with the Federal W-4 Form.  This direct link to the U.S. Department of Labor website is available for forms that are specific to your state.

Job Descriptions

 A job description is just that, a description of the duties and responsibilities expected of an employee by an employer!  While every facility is going to have unique job titles and descriptions, most job descriptions follow the format below:

  • Job functions and responsibilities
  • Job standards
  • Equipment used
  • Education and experience
  • Certifications or special qualifications
